Instruction for applying for a child’s Birth Certificate
And requesting for an Electronic Passport
** Remark : E-passport applicants needs to make an appointment prior to arrival
at the Royal Thai Embassy. BOTH parents need to accompany the child.
1. Document required:
The valid passports of father and mother (including two photocopies);
One passport-sized photograph of the applicant taken within the previous 6 months;
Full Birth Certificate of the applicant issued by the British authority which provides the correct parents’ names (including two photocopies);
Marriage Certificate of the parents (including two photocopies);
Divorce Certificate: If parents are divorced or do not live together, a confirmation letter or document certifying child custody (including two photocopies);
Father and mother’s Thai Citizen Identity Cards or House Registration Document (Alien father or mother exempted) (including two photocopies);
Please be informed that both father and mother are requested to give consent for their child to apply for a Thai passport by signing a consent form in person at the Thai Embassy.
Note: In case the child was born before 25 th February 1992, the applicant is advised to contact the Royal Thai Embassy in advance for getting additional instruction.
2. How to complete the application forms for a child’s Birth Certificate and Electronic Passport
Complete an application form for a child’s Birth Certificate in Thai, using the detailed reference of Birth Certificate issued by British authority i.e. forename, surname, hospital name and weight at birth (in grams) clearly and correctly;
In the application form for a child’s Birth certificate, box number 4.8, fill in the name of a person who notifies the birth which can be either father or mother of the child, but that person must be a Thai national. Indication of title of “Mrs/Mr” must not be given. For example: “ Marisa Smith”;
Complete an application form for Electronic Passport in Thai clearly and correctly, using the applicant’s surname and forename as they appear on the current Thai Citizen Identity Card ( Form 1) :
3. Fee and how to pay
The e-passport application fee is £25.00 GBP. The passport is valid for 5 years and contains 50 pages. It cannot be renewed.
Payment can be made by cash and postal order (which can be purchased at UK post offices, payable to: the Royal Thai Embassy, London). Please be informed that personal cheques and bank drafts are not accepted.
Should the applicants wish the Royal Thai Embassy to mail their new passports by post, a properly stamped and addressed envelope is required, as follows:
If the applicant resides in the United Kingdom, please enclose an SAE, stamped £1.50 GBP for Recorded Delivery, or £5.00 GBP for Special Delivery.
If the applicant resides in the Republic of Ireland, please pay an additional £6.00 GBP. Please be advised that only GBP sterling is accepted.
Note: Please be informed that you are required to provide your personal details i.e., your forename and surname, as they appear on your Thai Citizen Identity Card, Birth Certificate or House Registration document.
