In-family Adoption / relative adoption
or non-relative adoption

The following document requested to present to the official at the Child Adoption Center of Department Social Development and Welfare or the respective Social Development and Welfare Office in their home town are :

1. The birth certificate of the child to be adopted, including his or her housing registration and the child's photo.

2. The child's identity card and consent (in case that he or she is over 15 years old).*

3. Marriage certificate identity cards, housing registration of the child's parents and photos of the child's parents.

4. Consent by the child's parents (to be signed with the presence of authorized official).*

5. Divorce certificate or a court order giving consent on behalf of the child's parent for adoption (if any)

6. Death certificate of the child's mother or the child's father. If both of the parents died. The parents intending to be adoptor may apply to the court for an order allowing the adoption in lieu of giving the consent of the child's parents.

7. In case the child's parents have not registered their marriage, consent by the child's father or if the child's father could not be traced a statement confirmed by the authorized official at the District Office is also required.

8. Child study.

เอกสารที่ใช้ประกอบในการขอรับเด็กซึ่งเป็นบุตรติดคู่สมรสหรือญาติ

1. สำเนาสูติบัตร ทะเบียนบ้าน และรูปถ่ายของเด็กจำนวน 4 รูป

2. สำเนาบัตรประจำตัวประชาชนและหนังสือแสดงความยินยอมของเด็กที่จะเป็นบุตรบุญธรรม ในกรณีที่เด็กมีอายุเกิน 15 ปี ขึ้นไป

3. สำเนาทะเบียนสมรส บัตรประชาชนหรือบัตรข้าราชการ ทะเบียนบ้านของบืดามารดาของเด็กและรูป ถ่ายของบิดามารดาคนล่ะ 4 รูป

4. หนังสือแสดงความยินยอมของผู้มีอำนาจให้ความยินยอม (แบบ บธ. 6) และหนังสือแสดงความ
เห็นชอบของบิดาโดยพฤตินัย

5. สำเนาหนังสือสำคัญการหย่า และข้อตกลงการหย่าระบุผู้อำนาจปกครองเด็กหรือคำสั่งศาลให
้ความยินยอมแทนบิดารมารดาเด็ก แล้วแต่กรณี

ุ6. สำเนาใบมรณะบัตรของบิดาหรือมารดาเด็ก กรณีทั้งบิดามารดาเด็กเสียชีวิต ผู้ขอรับเด็ก ต้องร้องต่อศาลให้มีคำสั่งแสดงความยินยอมแทนบิดามารดาเด็กในการรับบุตรบุญธรรม

7. ในกรณีที่บิดามารดา ไม่ได้จดทะเบียนสมรสกัน ไม่สามารถติดตามบิดามาให้ความยินยอมได้ให้ มารดาเด็กไปสอบข้อเท็จจริง (แบบ ปค. 14) สำนักทะเบียนเขตหรืออำเภอที่มารดาเด็กมีภูมิลำเนา

8. รายงานการศึกษาประวัติเด็กที่จะเป็นบุตรบุญธรรม

เอกสารต้นฉบับจะต้องนำมาแสดงต่อเจ้าหน้าที่ศูนย์อำนวยการรับเด็กเป็นบุตรบุญธรรม กรมพัฒนา สังคมและสวัสดิการ หรือ สำนักงานพัฒนาสังคมและสวัสดิการจังหวัด ที่คนมีภูมิลำเนาอยู่* (มีทะเบียนบ้าน)
